Dr. Sunil K. Jain, Rehabilitation Unit Medical Direcotor

Dr. Sunil K. Jain is Crockett Hospital Rehabilitation Unit's Medical Director.  While a patient in the rehab unit, medical care is provided by Dr. Jain.

Dr. Jain is a physiatrist (Fizz-ee-AT-Trist), which is a medical physician who has specialized in Physical Medicine and Rehabilitation.

The patient's family physician may also come to see the patient while in Crockett Rehabilitation Unit.

Crockett Hospital Rehabilitation Unit’s mission is that our unit continues to be a community leader in providing education, resources, and medical and rehabilitation services to people with disabilities in order to optimize functional independence and provide an improved quality of life.  The Program Director works with the rehab staff to ensure that patients receive the best of care. 

Carolyn Wright, Case Coordinator

The first person that you are likely to meet from the rehab unit is our Case Coordinator, Carolyn Wright, RN BSN CRRN.

When a family member, case manager, or physician makes a referral, Carolyn completes a pre-admission evaluation, at no charge, to determine if Crockett Hospital Rehabilitation Unit can be of help to you.  She communicates with your insurance provider to ensure your insurance coverage.  Carolyn also communicates with the physicians and family members.

If the referral is out of town, Carolyn coordinates the transfer process from facility to facility.

Frances Allen, BSW Social Worker

Frances Allen is Crockett Hopsital Rehabilitation Unit's Social Worker.  Frances helps keep the lines of communication open between the patient, family and staff.  Frances works to obtain the equipment or services the patient may need at discharge.  Frances also follows up with the patient after discharge to make sure things are going smoothly at home.
