Registration Information

We want to ensure that you complete your registration as quickly as possible.

Centralized Scheduling
At Crockett Hospital all procedures are scheduled with a single phone call. Both physicians and patients benefit from the convenient, precise coordination of services and availability of accurate preparatory instructions. The telephone number for Central Scheduling is 931-766-3242.

Inpatient and Outpatient Registration
Registration for both inpatient and outpatient services is in the main lobby of the hospital from 6:00 AM - 6:00 PM, Monday through Friday, and in the emergency room evenings and weekends. It is important that you arrive on time because your physician may have scheduled special tests for you.

Crockett Hospital provides inpatient and outpatient care without regard to race, color, religion, disability or national origin.

Please bring the following items:

• All active medical insurance cards
• Social Security card
• Name, address, and telephone number of immediate family member
• Current list of all medications
• Living Will, if you have one

You may be required to sign the following:

• Consent for treatment
• Authorization to bill insurance

If you are a Medicare recipient, you will be required to sign:

• Medicare Secondary questionaire
• Medicare Rights Statement

Valuables
Crockett Hospital cannot be responsible for your valuables. We ask that you leave jewelry, large amounts of money, and other valuables home.

What to pack
You may wish to bring the following personal items: Nightwear, robe, robber-soled slippers, and personal toiletries.
